What Is Whiplash?

One of the most common injuries I see in my clinic is whiplash following a car accident. Unfortunately, it is also one of the most misunderstood—and frequently undertreated—injuries.

Whiplash is a soft tissue injury of the neck that occurs when the head and neck are suddenly forced forward and backward, often during a rear-end collision. This rapid motion places significant stress on the muscles, ligaments, joints, and nervous system.

How Whiplash Happens

During impact, the head is thrown forward and then violently backward in a fraction of a second. This mechanism strains:

  • Neck muscles (front and back)
  • Ligaments that stabilize the spine
  • Joints of the cervical spine
  • Upper back muscles
  • Jaw muscles (TMJ involvement is common)

If your head is turned at the moment of impact, the injury can be even more severe. This is called a lateral flexion whiplash injury, and it places asymmetrical stress on the spine and soft tissues.

While fractures can occur in severe trauma, the vast majority of whiplash pain comes from soft tissue injury—not bones.


Common Signs and Symptoms of Whiplash

Whiplash symptoms often do not appear immediately. Many patients feel “fine” at first, only to develop symptoms days later.

Common symptoms include:

  • Headaches
  • Neck pain and stiffness
  • Pain when turning the head
  • Jaw (TMJ) pain
  • Dizziness
  • Balance issues
  • Upper back or shoulder tension

Why You Should Not Ignore Whiplash

Soft tissue injuries do not always heal properly on their own.

If left untreated, whiplash can lead to:

  • Spinal degeneration
  • Ongoing nerve irritation
  • Persistent muscle tension patterns

Once pain decreases and mobility improves, targeted rehabilitation exercises are introduced to:

  • Stabilize the spine
  • Improve posture
  • Prevent reinjury
  • Maintain long-term results
